Payment Information:

At Cache Valley Pediatric Dentistry, we are proud to offer the finest and most up-to-date dental care for your child. We are dedicated to giving your child top care at reasonable and very competitive prices. We are a business that depends upon payment from our patients for services rendered to remain viable.  Therefore, financial responsibility on the part of each patient must be determined before treatment. To help you understand exactly how we work with you and your insurance company for reimbursement, the following has been provided as a complete list of all accepted forms of payment and all policies associated with payment in our office.

Insurance Billing and CoPays:

We will be happy to file your insurance claim starting on your first visit if we have received all of your insurance information. You will need to be prepared to pay any amount that is determined not payable by your insurance plan, such as deductibles and copay percentages.  Please understand that we cannot become involved in prolonged insurance negotiations.  If our office has not received payment from your insurance company within 60 days from the date of service, the balance in full becomes your responsibility.

Our office is a preferred provider for many insurance companies.  This means that we provide discounted rates on covered services. If we are not a preferred provider for your insurance company, we will accept payment from them, but we are not bound contractually to lower our rates.  We do our best to maximize the insurance benefits you are eligible to receive. You are responsible to know what your insurance covers and what it does not.  Your insurance company, not our office, is the best resource you have for determining what your coverage is. If your insurance plan coverage changes it is your responsibility to bring those changes to our attention.

Payment Options:

Payment is expected in full at the time of service.  We understand that financial difficulties arise and are sensitive to these matters.  If you have any questions regarding these payment options please don’t hesitate to call our office.

For our valued patients who are not covered by dental insurance, we offer an in-office loyalty program (see Cache Valley Smiles Loyalty Program below).

Care Credit:

Care Credit is a line of credit that is only available to be used for healthcare.  A short application which can be completed in our office is required, and we know within minutes what your available credit line is.  They offer up to 6 months interest free financing.

In Office Financing:

If Care Credit does not extend a line of credit to you, we offer In Office Financing at 18% interest. This option requires a credit card to be left on file in our encrypted system and regular payments to be made. If a payment is missed, financing becomes unavailable to you and your balance will be due in full in 10 days.


Cache Valley Smiles LOYALTY PROGRAM

The cost of health and dental care continues to increase each year. In an effort to encourage good dental health and alleviate rising dental costs, we have developed an In-House Dental Loyalty Program available to patients who cannot obtain dental insurance or find that our plan costs less than their current dental coverage plan.  You are not required to participate in this plan. We have discounted fees for those who are paying out of pocket for services and do not wish to participate. This plan is designed to help our uninsured patients achieve an exceptional standard of care, while offering the best value available for pediatric care in Cache Valley.

    6-11 years old:   $275/year or $24/month

   Multiple children discount: after first child is enrolled, each additional child per family is discounted $60/year or $5/month

                Check-Up 1:       Exam, bitewing x-rays, cleaning, fluoride

                 One panoramic x-ray every 5 years.

                  Limited Orthodontics as needed offered at a reduced rate.

No Annual Deductible

No Preauthorizations Required
No Waiting Period or Eligibility Periods for Major Treatment

Annual Enrollment Fee for each child is non-refundable
Annual Enrollment Fees are due at the first appointment of the year or can be paid early for the following year's enrollment. No exceptions.
Enrollment Date begins on the date the Enrollment fee is paid and will expire after 365 days.
Children in the same family can enroll at different times of the year.
All treatment must be PAID IN FULL at each visit to keep the plan in effect. No additional discounts or coupons may be applied to treatments.
The exams, bitewing, x-rays, and cleanings must occur withing the year of enrollment and cannot be carried over to the next year.
It is the patient's/parent's responsibility to make and keep appointments for his/her family members.
A $25 fee will incur for each missed appointment without a 24-hour advance notice.
Our program is not transferable to another party or uncovered child.
Participation cannot be combined with any other offer or dental plan.
All fees will be based on our practice's fees and not any other dental plan's fees.
